Who needs to complete this form?

This form must be completed by all non-citizen students not in F-1 or J-1 status.

Why do I need to complete this form?

This is in order to meet the requirements Eligibility Verification for Entitlements Act (EVEA). EVEA is a Tennessee law requiring that all students seeking a public benefit have valid status. These requirements must be met in order to receive state or institutional grants and scholarships. Please see One Stop Student Services for more information.

How do I complete the form?

Step One:

The e-form will be available to you through iVols. Log in to iVols following the instructions here.

Step Two:

Open

In the Document Upload section, you must upload a scan of the required documents.

Passport: This is a required document. Please upload a scan of your valid passport. We cannot clear the hold without this.

Visa: This is not a required document, but it may be necessary to establish lawful presence. Note, not everyone will have a visa. If you do have a visa, please upload a scan of the last visa you used to enter the country.

Form I-94: The form I-94 is a required document. Most people can pull their latest I-94 form at the Official Form I-94 website. Your form i-94 may be attached to another document (for example a form I-797A). If this is the case, upload the the entire document.

Other Documents: This is not a required document, but it may be necessary to establish lawful presence. If you have filed a permanent residency application or another application to adjust status, please upload the document here.

Step Three:

Open

If you are planning to study in your current status, Select “No” and submit the form. We will contact you if there are any issues with your documentation.

If you do need to pursue a change of status to F-1 student, select “Yes” and submit the form. You will also need to book a change of status consultation with us here.
